Needs updated

This app used to work very well before the previous update. Now it needs a major update to fix the problems it has. For one thing, its no longer very voiceover friendly. Very difficult to find the correct Field to fill-in with a visual impairment. Boxes dont line up correctly correctly with what youre trying to fill-in.
A delete button option should be created to allow the user to delete any old profiles.
Unable to insert physicians name and information from contacts.
This used to be a very good app like I said before. Hopefully it can return to that state.
